GROUP 1 TASK 3 CASE STUDY
Konstantina (“Connie”) Galanos is a 79-year-old widow living in a small regional city in South East Queensland. Born in Greece, Connie has a mid-range understanding of spoken English, although she only speaks Greek with her two daughters and one son. Her eldest daughter, Anna, lives next door with her family. Connie’s other daughter Steff lives in Brisbane and her son Chris lives in Sydney.
Mrs Galanos has a 20-year history of insulin-dependent diabetes mellitus and now lives with chronic renal failure and hypertension. Ten years ago, she underwent a hemicolectomy for bowel cancer and required a permanent colostomy, which she has self-managed up until about two years ago. She is known to the OPD (Outpatient Department) teams for colorectal, renal and diabetes care.
Formerly a regular attendee at her Greek Orthodox church, Connie is now confined to her home and relies on her daughter and her family, and friends from her church to support her to live at home.
She has had the same GP for the last 25 years. Her financial income is limited to the pension.
Connie was admitted to the general orthopaedic ward for a planned left lower leg amputation following a large, persistent and infected ulcer of her heel, and three gangrenous toes. She is visibly distressed despite Anna’s reassurances that she will be safe in hospital.
GROUP 2 TASK 3 CASE STUDY
Brendan Penn is a 25-year-old man living on the edge of a small rural town in South East Queensland where he has lived all his life. Although he has suffered episodes of anxiety since his adolescence, Brendan maintains good health. Brendan lives with his girlfriend Zoe in their small rental house, and his parents live in the town too. He works for the local butcher and Zoe currently is not in paid employment following an argument with her employer in the next town. Brendan does not visit a general practitioner – whenever he has been unwell, he has gone the emergency department at a nearby regional city.
Three weeks ago, Brendan was admitted to hospital following a severe motorcycle accident where he sustained a compound fracture of the right femur and closed fractures of the right radius and ulna. Following surgery, his leg wound became infected and required IV antibiotics. The wound is now granulating and requires regular dressing. His arm remains in a sling.
You have nursed Brendan on several shifts at the hospital and now plans are being made for his discharge home. You have noticed a change in Brendan’s behaviour. He has gone from being withdrawn and tearful to suddenly agitated and angry.
